SUMMARY

The Nurse-Family Partnership Nurse Home Visitor is responsible for providing comprehensive nursing services to women and their families eligible for the NFP Program. Nurse Home Visitors are pivotal in transforming the lives of vulnerable babies, mothers, and families by implementing and maintaining fidelity to the model.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
  • Adhering to Nurse Family Partnership model guidelines of home visitation, provides home visits to women and their families eligible for the program, carrying a case load.
  • Develops therapeutic relationships with women and their families and aids with establishing and attaining target goals and outcomes through education, and referral resources.
  • Assesses physical, emotional, social, and environmental needs of women and their families.
  • Monitor maternal and child health.
  • Nurse Home Visitors develop and maintains community relationships to support client referrals, consulting and collaborating with other professionals involved in providing services to women and families, and as requested, promotes public awareness of the program.

EDUCATION, LICENSURE and EXPERIENCE
  • Must be a registered nurse with a minimum of a Bachelor's degree in Nursing (BSN) with a current RN license (in good standing). Master's degree in Nursing (MSN) or health related degree desirable.
  • Experience in public health, community, and/or maternal-child health, required.
  • Current CPR required.
  • Home visiting experience preferred.
  • Bilingual (Spanish/English), desirable.
QUALIFICATIONS
  • Ability to work cooperatively and respectfully with others, including people of diverse cultural backgrounds and ethnic groups.
  • Demonstrate ability to establish and maintain effective working relationships with clients, co-workers, and community.
  • Ability to help others learn through a variety of methods· Strong oral and written communications skills.
  • Knowledge of community resources.·
  • Ability to work independently as well as a member of the team.
  • Experience with computers, including Microsoft Office.·
  • Must have valid driver's license, which meets the requirements of the agency's insurance carrier.
  • Ability to work flexible hours, including nights and weekends, as needed.
