PRN CCFT Case Manager

The PRN CCFT Case Manager will:

  • Act a liaison with community agencies and families to advocate for the rights and preferences of clients and to facilitate the treatment process.
  • Provides advocacy, linkage, and referral services
  • Maintains appropriate chart records
  • Provides mental health assessments, evaluations, and consultative services
  • Facilitation of groups
  • Completes all documentation in a timely manner
  • Participates in treatment team meetings with a psychiatrist
  • Participates in IEP and other school related meetings as need or as requested by families
  • Interfaces professionally with other agencies involved in the ecology of the family
  • Provides therapeutic support and crisis intervention, including on-call crisis services 24/7

Schedule: This position is "As Needed" and may require irregular hours. This position provides services Monday-Friday during regular business hours and also includes an on call component during which the clinician would be available to respond to after-hours crises by phone or in person as needed.

Travel: This position requires utilizing a personal dependable vehicle to conduct Center business. Maintaining a dependable vehicle and certified driver status is a condition of employment.

Equipment/Technology: This position requires the use of computer skills with an electronic medical record (EMR), and telehealth/televideo services may be offered as part of the service delivery. Staff will be expected to maintain work with a Center cell phone and computer.

QUALIFICATIONS - PRN CCFT Case Manager

Education:

  • Bachelor's degree in a health-related field of counseling, psychology or social work, or must be licensed as a nurse (LPN or RN). LPNs must have fifteen (15) semester hours of behavioral health and one (1) year of behavioral health care expectations.
  • Candidates with a Bachelor's degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen (15) college-level semester hours of course work in behavioral health and at least one (1) year of work experience in the behavioral health setting.

Experience / Knowledge:

  • Must have course work and or experience in the areas of cultural diversity, human development, etiology and treatment of mental illness, alcohol and drug abuse, physical and sexual abuse, suicide, and intellectual/developmental disabilities.
  • One year experience preferred.
  • Experience working with children is preferred although applicants may have other skills and experiences that could accommodate this position.

Physical:

  • Minimal exposure to biological hazards.
  • Frequent exposure to unpleasant odors.
  • Hearing of normal and soft tones.
  • Close eye work.
  • Valid driver's license.
  • Frequent sitting, standing, walking, bending, stooping, and reaching.
  • CPR and First Aid certification required.
  • Required to be certified in and adequately implement verbal de-escalation techniques.
  • Applicants should be able to exercise sound judgement under pressure.
  • Applicants should possess the ability to relate to individuals and families of varied ethnic and cultural backgrounds, ages, educational levels, and economic circumstances with respect and dignity.
  • Effective communication skills, strong time management, and organizational skills are necessary to manage day to day job responsibilities.
  • Excellent verbal, written, and computer skills are necessary in order to work effectively and collaboratively with clients, collaborating agencies, and community professionals, as well as maintaining proper documentation.
  • Clinical staff members must possess reliable transportation, the ability to acquire an F endorsement on driver's license, and transport clients.
