Social Worker III

This is the proficiency level III classification in the Social Worker series. The work involves the application of social work techniques to the practical and emotional needs of patients. Incumbents in this job classification perform social services for inmates, clients, or patients of state hospitals, clinics, and institutions, or other settings, both medical and psychiatric. The work also includes routine and standard services for patients and patients' families. Emphasis of work is on the independent performance of case management/casework services; and assisting or consulting other professionals with evaluating and coordinating treatment plans in complex cases. Incumbents may participate in staff and administrative planning. General professional supervision is received from a social worker in a higher classification or from an administrative official.
Examples of Work:
Examples of work performed in this classification include, but are not limited to, the following:
Interviews clients, client groups, offenders, and others to obtain extensive social, developmental, psychological, medical, family, educational, and employment background information.
Determines clients' strengths and needs using the Client's Functional Assessment (CFA) to assist clients in achieving his/her goals.
Collaborates with other professionals to evaluate the mental and/or physical condition of clients; determines services required to meet clients' needs; and coordinates and manages treatment plans for patients.
Prepares and maintains case history records.
Modifies treatment plans according to changes in clients' status.
Serves as a Case Manager, ensuring that the residents' Individual Service Plan is correctly implemented and that all other disciplines are monitoring and measuring their active treatment objectives regularly.
Counsels patients and family members on adjusting to routines and life at the facility and outside the facility.
Assists families in understanding medical terminology.
Coordinates services among agencies contributing to patients' welfare; and schedules meetings with other professionals to ensure that clients are placed in the most appropriate setting.
Assists clients and individuals with receiving and sending mail and making phone calls to parents or guardians.
Assists clients in adhering to treatment plans, such as setting up appointments, arranging for transportation to appointments, providing support, meeting daily needs, and completing applications for benefits from state, federal, or private agencies.
Assists patients by coordinating Health Insurance Portability Accountability Act (HIPAA) related requests concerning access to medical records.
Updates and maintains contact information of parents and/or guardians.
Conducts admission and discharge interviews, and maintains summary reports.
Coordinates home visits (including notifying pharmacy for medication, and resident living for clothing and supplies).
Provides families with workers in the home to assist with medical and/or respite needs with their child and or adult family member.
Makes telephone calls and conducts face to face home visits with individuals and families to ensure services continue to be effective and appropriate.
Performs related or similar duties as required or assigned.
Minimum Qualifications:

EXPERIENCE/EDUCATIONAL REQUIREMENTS:
Education/Licensure:
Must be licensed to practice Master's level social work (LMSW or above) in the State of Mississippi;
AND
Experience:
One (1) year of experience related to the described duties;
OR
Education/Licensure:
Must be licensed to practice Bachelor's level social work (LSW) in the State of Mississippi;
AND
Experience:
Two (2) years of experience related to the described duties.
Documentation Required:
Applicants must attach a valid copy of his/her current social worker license.
Essential Functions:
Additional essential functions may be identified and included by the hiring agency. The essential functions include, but are not limited to, the following:
1. Conducts admission interviews to assess clients individually, in families, or in groups.
2. Prepares social histories to determine which services are required to meet patients' needs.
3. Collaborates with other professionals to coordinate clients' treatment plans.
4. Monitors, evaluates, and records clients' progress regarding treatment goals.
5. Counsels patients/clients/family members concerning the adjustment to treatment plans and/or daily routine.
6. Conducts discharge interviews and prepares discharge reports.
