    Position description

    As a critical member of a multidisciplinary team, you'll use your well-developed skills as a Patient Care Associate to assess acute care patients and deliver evidence-based care to patients in their homes as well as the Eau Claire hospital.

    The aide assists patients with delegated tasks of direct and indirect patient care, which includes providing personal care and assistance in activities of daily living. In the home environment, these services are included in a written plan of care prepared by a health care professional and signed by a physician. The aide works under the direct supervision of a Registered Nurse and/or when appropriate a Physical, Occupational or Speech Therapist under the delegation of their License. When working within the hospital campus, under the direct supervision of a registered nurse, the aide carries out designated activities which support nursing practice that are within those functions limited by law to unlicensed nursing personnel. This includes assisting patients with activities of daily living, providing basic nursing care to patients and assisting in the maintenance of a safe and clean environment. Assists RN with procedures and treatments that require competence in advanced technical and manual skills.

    Qualifications

    High school graduate or equivalent within one year of hire.

    Additional qualifications

    Minimum of six months healthcare related experience preferred. One year of related experience is highly preferred.

    Ability to read and communicate effectively in English. Flexibility (works flexible hours, which include days, evenings, nights, holidays, and weekends). Basic computer skills and experience with other technological devices (i.e. smart phones, tablets, etc.). Verbal and written communication. Promotes team building. Self-directed. Able to adapt to unpredictable situations. Able and willing to drive in all weather conditions and cover service area. Able and willing to work with all populations served.

    License or certification

    Meets Federal and State guidelines required to work as a Home Health Aide in a certified licensed Home Health agency and Certified Nursing Assistant on State of Wisconsin Nurse Aide Registry or Current Wisconsin EMT (Basic, Intermediate or Paramedic) certification. Maintains current Basic Life Support (BLS) for Health Care Providers from one of the following programs: American Heart Association or American Red Cross.

    Valid drivers license, reliable transportation, the ability to transport patients and acceptable driving record required.

    A driving record will be considered unacceptable if it reports any of the following:

    • Any combination of motor vehicle accidents and moving violations that total four or more incidents within the last 36 months.
    • One or more of the following citations or convictions within the last 5 years:
      • Driving while intoxicated
      • Driving under influence of drugs
      • Operating a motor vehicle during a period of license suspension or revocation
      • Reckless driving
      • Hit-and-run
      • Any other violation that indicates inability to drive safely
    • Driving restrictions that would conflict with job duties (e.g., restricted driving hours).
    • A driving citations or convictions considered more serious than the above (e.g., causing bodily harm/felony level).

    Compensation Detail

    Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $15.24 per hour.

    Schedule details

    Rotating 8 and 12 hour shift lengths, based on patient care needs.

    Weekend schedule

    12 hour rotating every 3rd weekend.
