      The RN is an active member of an assigned household who is committed to providing residents with assistance and care in all aspects of their daily life. Promotes positive personhood, participation at the highest possible level, quality of life and the overall functioning of the household. The RN is responsible for providing clinical leadership to the household including defining, planning, organizing, directing, managing, implementing, and evaluating nursing services provided to Community residents, in addition to dispensing medications, and communicating with the primary physician, family, other care staff, etc. regarding resident issues and needs.
      Essential Functions Include the Following:
      • RN's assume direct responsibility and accountability for the nursing care of the Community residents under his/her care. Our Registered Nurses supervise nursing personnel including LPNs and Resident Assistants.
      • Works with other household members to maintain functioning of the household including laundry, meal planning/preparation/serving/cleanup, household cleaning, etc., according to resident preferences.
      • All RN's provide opportunities for all residents to participate in the daily life and decision-making of the household in meaningful ways. Respects the resident?s suite and household as home and assists the residents in feeling in charge of everything that happens within.
      • RN's provide assistance with and opportunities for resident engagement. Continually seeks opportunities to meet resident?s psychosocial needs through both planned and spontaneous actions. Seeks opportunities throughout the day to facilitate spontaneous activities of the resident's choice and functional level. Our Registered Nurses supports and encourages residents to participate in self-care and assists with bathing, dressing, grooming, dental/mouth care, bowel and bladder care and other personal & hygiene care. Provides and documents patient/resident/family education.
      • Answers resident calls for assistance via pager system.
      • Assists in the preparation, serving and clean-up after meals. Assists community in ensuring that the resident?s nutritional status is maintained, food preferences honored and adherence to the resident?s prescribed diet where appropriate. Also provides opportunity for between meal, bedtime & other snacks and beverages as requested and preferred by the resident.
      • RN's greet new residents upon admission, orienting them and their families to the community. RN's complete required admission records collaborates in the development and implementation of an interim plan of nursing care on a timely basis.RN's Accurately administers and documents medications and treatments. Ensures proper check-in, storage, and destruction of all medication, including narcotics.
      • Our Registered Nurses communicates and collaborates with attending physician and/or consulting specialists. Accurately transcribes and implements authorized healthcare professional?s orders. Notifies attending physician, family and members of the neighborhood team when there is a significant change in the resident?s condition.
      • Initiates and maintains the residents? clinical records in accordance with procedures. Completes transfer and discharge records as needed along with other required forms, reports, evaluations, studies, charting, etc. Provides and documents observations and delivery of care according to established guidelines. Evaluates responses to interventions and the effectiveness of the plan of care.

      To Qualify for this Position:
      • Current license as RN in the state of Illinois in good standing with the Department of Professional Regulations.
      • CPR certification required.
      • Minimum six (6) months experience in a skilled nursing or assisted living facility preferred.
      • Knowledge of nursing regulations governing long-term care.

      1st Shift: 6:30am - 3pm
      2nd Shift: 2:30pm - 11pm; $1.25 differential
