      The LPN is an active member of an assigned household who is committed to providing residents with assistance and care in all aspects of their daily life. Promotes positive personhood, participation at the highest possible level, quality of life and the overall functioning of the household. The LPN is responsible for providing clinical leadership to the household including dispensing medications, observing resident?s conditions and communicating with the primary physician, family, other care staff, etc. regarding resident issues and needs.
      Essential Functions Include the Following:
      • Our LPN's work with other household members to maintain functioning of the household including laundry, meal planning/preparation/serving/cleanup, household cleaning, etc., according to resident preferences.
      • LPN's provides opportunities for all residents to participate in the daily life and decision-making of the household in meaningful ways. Respects the resident?s suite and household as home and assists the residents in feeling in charge of everything that happens within.
      • Our LPN's supports and encourages residents to participate in self-care and assists with bathing, dressing, grooming, dental/mouth care, bowel and bladder care and other personal & hygiene care.
      • Our LPN's assists in the preparation, serving and clean-up after meals. Assists community in ensuring that the resident?s nutritional status is maintained, food preferences honored and adherence to the resident?s prescribed diet where appropriate. Also provides opportunity for between meal, bedtime & other snacks and beverages as requested and preferred by the resident. Answers resident calls for assistance via pager system.
      • Household LPN's greet new residents upon admission, orienting them and their families to the community. Completes required admission records, collaborating in the development and implementation of an interim plan of nursing care, consulting with RN as needed regarding nursing evaluation.
      • All Licensed Practical Nurses are responsible for medication administration and accuracy of medication administration records; ensures proper check-in, storage, and destruction of all medication, including narcotics.
      • Our LPN's consult with attending physician and/or consulting specialist as needed for treatments, medications, rehabilitation, etc. Accurately transcribes and implements authorized healthcare professional?s orders.
      • Our LPN's initiates and maintains the residents? clinical records in accordance with procedures. Provides and documents observations and delivery of care according to established guidelines. Completes transfer and discharge records as needed along with other required forms, reports, evaluations, studies, charting, etc.
      • Our LPN's are responsible for notifying the attending physician, family and members of the neighborhood team when there is a significant change in the resident?s condition.

      To Qualify for this Position:
      • Current license as a Licensed Practical Nurse in the State of Illinois in good standing with the Department of Professional Regulations required.
      • CPR certification required within first thirty (30) days.
      • Minimum six (6) months experience in a skilled nursing or assisted living facility preferred; some experience with dementia care preferred.
      • Completion of food service sanitation (ServSafe) program required within six (6) months of hire date; certification preferred.
      • Knowledge of the information and techniques needed to assess and treat human injuries and diseases. This includes symptoms, treatment alternatives, drug properties and interactions, and preventive health-care measures.
