Clinical Coordinator Full-time

Under the agency?s policies and professional requirements, the Master Level BHT will provide services that include counseling, education, prevention, intervention and behavioral care support. Our Masters level BHTs will provide intensive Case Management and Care Coordination for individual and families. Our Community- Based Therapy Sessions are structured, goal-oriented and time-limited sessions in the natural environment of individual and family who need assistance recovering from trauma, emotional abuse, and neglect. Our Counseling team includes peers support, care coordinators and health care advocates. Clinical Coordinators will ensure that our services are aligned with Arizona?s Principles of Person-Centered Planning. Clinical Coordinators ensure therapeutic environments and treatment services are being met on a daily basis, ensuring timely submission of service paperwork. Clinical Coordinators will maintain a caseload of 15-18 members.

Major Responsibilities and Related Tasks:

Direct-Line Services

  • Complete initial contact to referred individual and families.
  • Complete an assessment with the Individual and family to develop appropriate/necessary recommendations for future success.
  • Provide necessary resource and support system development.
  • Provide counseling one to two times weekly.
  • Provide crisis intervention as necessary.
  • Participate in case conferences, and court appearances with the individual as requested.
  • Provide family advocacy as needed.
  • Provide case management and coordination of services with other health care providers.
  • Responsible for preparing monthly and quarterly reports
  • Responsible for discharge planning.
  • Provides direct clinical services to clients and their families and issue-specific or educational groups (parenting skills, anger management, substance abuse interventions)

Position Responsibilities:

  • Conducts behavioral health screenings, initial assessments, and on-going treatment and intervention.
  • Clinical Coordinator ensure that Treatment Plans are complete and clear of goals and objectives.
  • Clinical Team meets weekly to ensure that all Treatment Plans and Service Authorizations are updated and compliant.
  • Responsible for establishing trusting relationships with members and their families while providing general support and encouragement
  • Providing ongoing follow-up, basic motivational interviewing and goal setting with members/families
  • Follow-up with members via phone calls, home visits and visits to other settings where members can be found.
  • Provides clinical counseling services to members, including individual and group therapy.
  • Participates in ongoing training in behavioral health issues and provides training to PCs, BHTs, Mentors, BHT Leads, in behavioral health topics including counseling skills and confidentiality, medication monitoring and misuse, substance abuse, wellness, rehabilitation, recovery and self-sufficiency.
  • Assists in the assessment, planning, development, and implementation of psychosocial programs in compliance with ADHS and contract stakeholders
  • Conducts visits/treatment, phone, or electronic communication including pertinent medical and/or psychosocial history, diagnoses, treatment plans, goals, and progress and other pertinent information in accordance within the organization?s standards on documentation in the Electronic Medical Record within 48 hours and in compliance.
  • Completes intake and assessment and delegates to designated Program Coordinator ?of new members in the agency.
  • Establish, enforce and conduct clinical team meetings within your department or program service responsibilities.
  • Provide clinical assessment of members related to psychosocial programming,assist, support and manage Action Plans
  • Ensure each Member receives appropriate care in accordance with the Member's treatment plan, under the supervision of a Behavioral Health Professional

Minimum Requirements:

  • Masters Degree required: In a related field; however, counseling and/or substance use preferred
  • Minimum of at least 1year experience in the behavioral health and/or medical field
  • Care Giver experience preferred
  • Clean Driving Record
  • CPR/1st Aid
  • Clear TB Test
  • Level 1 Finger Print Clearance Card
  • Pass background check
