Personal Care Providers Full-time

Community Partners offers In-Home Support program. This program provides aid to families with clients aged three to twenty-one. Our amazing team is looking for Personal Care Providers (PCPs) who have flexible hours, are self-motivated, and are excited to make a positive difference in others? in the community. We are seeking to fill the following Personal Care Provider positions:

Job. #656: Personal Care Provider to work with an eleven year-old boy with Autism who lives in Strafford County. As his PCP, you will work on personal care goals around activities of daily living, both in the family home and in the community. Experience working with individuals with Autism is preferred but is not required as the family will provide training. PCP must be able to occasionally lift/assist client with transfers.

Hourly rate is dependent upon experience. The family is looking for a PCP for Wednesdays, Thursdays, and Fridays from 2:30 PM to 6:30 PM. Hours/days may be flexible. Additional information on this temporary position will be provided during the interview.

Job. #335: PCP to work with a 16-year-old young man with Autism from Farmington. The client enjoys staying busy, recreational opportunities, and music. He requires constant supervision to ensure safety, proper socialization techniques, and emotional regulation. Child is energetic, bubbly, boisterous little boy who loves to be around people and make new friends. As his PCP your responsibilities would include, but are not limited to, working on activities of daily living goals, increasing community and socialization skills, increasing meal preparation skills, increasing safety awareness, assistance with recreational activities (swimming and horseback riding), and personal care assistance (i.e., dressing). This 12-hours/week (Wednesday, Thursday, Friday: 3:00 PM to 6:00 PM; and school vacations) position offers a wage of $12/hour.

Job. #375: Rochester- Looking for after school/early evening year-round care for 16 year old young lady with Down Syndrome and Autism. Client has high energy and enjoys swimming, jumping rope and being out in her community. She requires 24/7 supervision to ensure safety and prompting to complete Activities of Daily Living (ADLs). Provider will be responsible for working on skill-building goals, such as personal hygiene tasks, independence with meal preparation and safety awareness in the community.

Job 524: The PCP will work with a sixteen-year-old young lady in Barrington, NH. As her Personal Care Provider, you will work with her on activities of daily living and communication. The client has a developmental disability and is nonverbal.

Experience in working with individuals with disabilities is preferred. Ability to assist with physical transfers and help with toileting is required. This Saturday position (specific hours to be discussed during interview) offers a salary starting at $12/hour.

Job 095: Are you interested in providing approximately 8-12 hours of Respite coverage a week, during the early evening hours and over the weekend? We are looking for someone with experience working with people who have Intellectual Disabilities, and working with people who may have communication issues leading to possible behavioral challenges. We are currently looking for someone to work with a fun loving, friendly young woman. Your time will include providing the services and supports needed in order for her to experience participation in requested/recommended local activities and activities of her choosing. As a Respite staff you will work with the Family in setting up scheduled Respite hours.

Job 706: Family in Dover seeking a Respite provider for their 16-year-old son with a Developmental Disability. A provider is needed several times a week, in the afternoon. This family is looking for an individual who is reliable, outgoing and has experience working with individuals with a Developmental Disability. Provider would bring client out in the community, which allows client a chance to learn valuable life skills, practice his behavioral management, emotional regulation and learn appropriate executive skills. For activities, client enjoys fishing, kayaking, art, and simply being active and in the outdoors.

While experience working with individuals with a Developmental Disability is not required, it is preferred.

Job 643: Dover -Family in search of a Personal Care Provider for their 4 year old son with Downs Syndrome. Child is an energetic, bubbly, boisterous little boy who loves to be around people and make new friends. Responsibilities would include:

  • Working on ADL (activities of daily living) goals
  • Increasing independence with mobility
  • Increasing fine and gross motor skills
  • Increasing independence with communication skills
  • Personal care assistance ie/toileting and hygiene tasks

Must be CPR certified or willing and able to become certified with Community Partners? training. Driver?s license and proof of vehicle insurance is also required. Interest/experience in the human services field, working with children and an understanding of disabilities is encouraged.

Local travel is required for the position, candidates must have a valid driver?s license, clean driving record, proof of adequate vehicle liability coverage, and reliable transportation.

Positions require a pre-employment background check which includes, but is not limited to, criminal and driving records are required. A High School diploma or equivalent is required for all positions.

In the Self-Directed Services program, families in Strafford County manage their own services (including staffing) for a family member who has a developmental disability or a traumatic brain injury. The employee will work out of the family home and the family is the Managing Supervisor with Community Partners as the Employer of Record.
